Fills all your criteria but I can't comment on the price. He had a form focused design but it did feature flat & pitched roof sections.
He wrapped it in another 50mm EPS to remove any thermal bridging achieving about 0.10 W/m2K overall.
Out of interest, what system did you choose in the end?
